What the Guthrie County Hospital Social Worker Can Do:
Medical Social Services can help individual’s couples and families cope with the social, psychological, cultural and medical issues resulting from an illness. The social worker also helps patients fully utilize medical care and services by:
Providing and explaining health care resources to patients and their families.
Helping plan for post-hospital patient needs through the arrangement of services at another facility or at home.
Examining with patients and families disease process and the social/environmental risk factors.
Explore and arrange referrals to health care resources and follow up care.
Understanding the social, cultural and religious variables that contribute to patients’ responses to illness and their use of health care resources.
Addressing and assisting with Advanced Directives.
Help with Long term care issues.
Individual Counseling.
